It is with deep sorrow that we announce the peaceful passing of Diane S. Tavares, age 78, of Fallon, Nevada, who passed away on Tuesday, November 18, 2025, at 10:15 p.m. She spent her last days surrounded by her sons, close family, and loved ones.
Diane was born on June 5, 1947, in Cortez, Colorado, to James Lynn Slade and Mary Alice Slade. She was one of three children and valued family above all else. Diane was a devoted daughter, sister, wife, mother, sister-in-law, aunt, grandmother, and great-grandmother. She graduated from BYU Hawaii. Diane taught in Maui before moving to Las Vegas, Nevada, and changing careers to social services for the state of Nevada, where she dedicated her career to helping others.
Diane married Gary Tavares in 1973 in Honolulu, Hawaii; together they have three sons, 10 grandkids, one great-grandchild, and another on the way. She will be remembered for her loving nature, her love of antique collectibles, her sense of humor, her travels, and her enjoyment of time with her grandchildren.
Diane is survived by her three sons and two daughters-in-law, Harrison Tavares, Jared Tavares (Tyler), and Jonathan Tavares (Erin); grandchildren, Taylor Tavares, Alexus Crabb, Hannah Tavares, Donovan Tavares, Shaelyn Tavares, Allison Tavares, Jayden Tavares, Cassandra Tavares, Kennedy Tavares, and Trenton Tavares; great-grandchild, Damon Crabb. Siblings: Jim Slade (Karen), and Dean Slade (Susan). Diane is preceded in death by her parents and her spouse, Gary Spouse.
A memorial service will be held Saturday, January 17th, 2026, at 1:00 p.m., at the Church of Jesus Christ of Latter-day Saints, 750 W Richards Street, Fallon, Nevada. Family and friends are invited to attend and celebrate Diane’s life. A reception will be held immediately after the service in the church's reception hall.
In lieu of flowers, donations may be made to Circle of Life Hospice.
The family wishes to extend heartfelt thanks to the hospitals and staff, hospice caregivers, and all who supported her on this journey.
Diane will be dearly missed and forever remembered.
